Turning 65 in 2027? Your Medicare Enrollment Roadmap

Senior reviewing a roadmap to understand Medicare enrollment steps

If you’re turning 65 in 2027, now is the time to start preparing for Medicare. Enrolling at the right time ensures you get the coverage you need without late penalties or gaps in care. At NW Senior Insurance Solutions, we specialize in guiding Medicare-eligible individuals through the enrollment process, helping you make informed decisions about your healthcare options. If you live in Clark or Cowlitz counties in Washington, Multnomah, Clackamas, Washington, Marion, or Lane counties in Oregon, or Maui County in Hawaii, this guide is designed specifically for you.

When to Enroll in Medicare?

Your Initial Enrollment Period (IEP) begins three months before your 65th birthday month and continues for three months after. For example, if your 65th birthday is in June 2027, your IEP runs from March 1 to September 30, 2027. If your birthday is on the first of the month, the window starts and ends a month earlier (February 1 to August 31 for a June 1 birthday). If you turn 65 in January 2027 on any day except January 1, your window runs from October 1, 2026, through April 30, 2027. Premium-free Part A generally starts the first day of the month you turn 65. For Part B, signing up during the three months before that month starts coverage the first day of that month; signing up during that month or later in your Initial Enrollment Period starts it the month after you sign up. If your birthday is on the first of the month, Medicare counts the month before your birthday as the month you turn 65.

Understanding Your Medicare Options

Medicare is made up of different parts, and selecting the right coverage depends on your healthcare needs and budget:

  • Medicare Part A (Hospital Insurance): Covers hospital stays, skilled nursing facility care, hospice, and some home health services. Most people qualify for premium-free Part A.
  • Medicare Part B (Medical Insurance): Covers doctor visits, outpatient care, preventive services, and some home health care. Part B has a standard monthly premium, and people with higher incomes pay more.
  • Medicare Advantage (Part C): A private insurance alternative to Original Medicare that often includes additional benefits like vision, dental, and prescription drug coverage.
  • Medicare Part D (Prescription Drug Coverage): Helps cover the cost of prescription medications and can be added to Original Medicare.
  • Medicare Supplement (Medigap): Helps cover out-of-pocket costs associated with Original Medicare, such as deductibles and copayments.

Planning for 2027

Medicare’s Annual Enrollment Period (October 15 to December 7) is for people who already have Medicare. If you’re new to Medicare, your Initial Enrollment Period is the window that counts, so you may be choosing coverage at a different time of year than friends who are already enrolled.

Some plans in our area are changing for 2027. Providence Health Plan will not offer Medicare Advantage plans in 2027, so if you were expecting to join one, read what the change means. Medicare Advantage networks can also change from year to year, so check your doctors before you choose a plan: Oregon, Clark County, or Maui and Hawaii.

Key Deadlines and Avoiding Penalties

Missing your Initial Enrollment Period may result in penalties or delayed coverage. If you’re still working and covered under an employer plan, you may qualify for a Special Enrollment Period (SEP) that allows you to enroll in Medicare later without penalties.
Otherwise, enrolling on time ensures you get the benefits you need when you need them.
